An almost 2-year-old health program is seeing lots of satisfaction in Oxford County.
OXFORD COUNTY - Oxford County is seeing satisfaction in a health program.
A Remote Patient Monitoring Program was launched in August 2022 and has seen a satisfaction rate of 97 percent as of April of this year. Superintendent of Community Paramedicine, Jamie Walter explains the program.
"The Remote Care Monitoring Program is a way that we can bring medicine to individuals in their home, by way of monitoring their assessments."
Walter says there are kits including a blood pressure machine, scale, and an oxygen saturation probe. The equipment is used to monitor the biometrics of the patients and sends alerts, which the community paramedics could respond to if they feel they are outside the normal parameters. Walter says how people become a part of the program.
"We take referrals through care coordinators through hospitals, historically we have taken them from paramedics working the 9-1-1 side who may come across individuals in the community, and the most common referral pathway comes from family physicians."
Remote monitoring patients range in age from 22 to 98 years old and typically have moderate to severe chronic diseases, such as chronic obstructive pulmonary disease (COPD), congestive heart failure, hypertension and diabetes.
